University Hospitals Birmingham is seeking a motivated Continuing Healthcare (CHC) Assessor to join our Community Services team in Solihull. This is an exciting opportunity to play a key role in assessing, reviewing and coordinating NHS Continuing Healthcare and Funded Nursing Care in line with the National Framework. You will work collaboratively with patients, families, community teams, acute services, social care and care providers to ensure timely, high-quality decision-making and positive patient outcomes. As a senior autonomous practitioner, you will undertake complex assessments, support discharge planning, complete Decision Support Tools, facilitate multidisciplinary working and contribute to service development, quality improvement and staff education. The role offers the opportunity to influence pathways, improve patient experience and support delivery of integrated neighbourhood care. We are looking for a registered nurse with substantial post-registration experience, specialist clinical knowledge, excellent communication skills and a strong understanding of NHS Continuing Healthcare.
