We are looking for a substantive Band 6 registered professional Mental Health professional to be part of the Learning Disability Service in Sirona. The post holder will be employed by AWP and receive management supervision within AWP but be on an honorary contract for Sirona and fully embedded into the Sirona service

Your role will involve CPA co-ordination, holding a caseload of clients who may have complex presentations, providing consultation and mental health advice to members of the wider Community Learning Disability Team and mainstream services.

You will work mainly across the Bristol area and will work with service users in a variety of settings. Some travel to further locations may be required to attend CPA / CTR meetings and support the timely discharge of service users in out of area hospital placements in accordance with the Transforming Care agenda.

The ideal candidate will have the ability to understand how mental illness in people with Learning Disabilities may be different from a mainstream presentation. The ability to implement “reasonable adjustments” in the assessment, planning and delivery of mental health care is an essential part of this role.

An ability to liaise with mainstream inpatient services about what unique challenges someone with a Learning Disability might face in hospital is an important aspect of this role. You will support service users where possible to access mainstream services and will provide advice and consultation to these services.

### Main duties of the job

You will be working closely with Consultant psychiatrists to monitor the effects of psychotropic medication. You will have the ability to work in a collaborative, pro-active and preventative way to reduce the need for admission to hospital and deliver interventions to promote recovery and wellness. You will be one of three nursing practitioners who support the needs of people with Learning Disabilities who have additional mental health needs.

### Detailed job description and main responsibilities

The Sirona Adult Learning Disability Health (ALDH) Service is a centre of excellence for specialist learning disability health needs. We cover 3 localities where multidisciplinary teams enable our service users to access health services through direct care, liaison with other services and offering support and training on how to work with people who have a learning disability and how to make reasonable adjustments. The service prides itself on delivering high quality support to service users with a variety of physical and mental health issues alongside their Learning Disability. All staff are required to use a range of communication and listening skills to work effectively with our service users. Where necessary you will be required to work at other bases or work remotely from home.

This post represents a key opportunity to contribute to the transformation of the Mental Health Nurse role within the Learning Disability service. The role is evolving to meet the complex and changing needs of individuals with learning disabilities and co-occurring mental health conditions across Bristol, North Somerset and South Gloucestershire.

## Person Specification

### Education/Experience/Skills

**Essential**

- Diploma level/Degree in relevant health/social care profession, eg RMN, OT, Social Worker
- Demonstrates substantial post-registration experience gained working in a range of front-line adult mental health services, undertaking assessment and delivering interventions
- Current relevant registration with the NMC or HCPC and commitment to CPD.
- Demonstrates a developed understanding of the recovery principles, and the role of secondary mental health services in the delivery of care
- Demonstrates substantial experience of assessing risk and developing risk management strategies

**Desirable**

- Experience gained in a wide variety of settings
- Relevant post-registration qualifications, such as Thorn, Non Medical Prescribing, CBT, AMHP, Brief Interventions etc
