Are you ready to become one of our Rapid Practitioners in physical health service? We are looking for an enthusiastic Registered General Nurse or Paramedic who can help the CIS make Hammersmith & Fulham a great place to grow old.

You will be key in developing the service within the CIS to deliver effective pathways of care through hospital to community and in building relationships with our hospital and community partners. We will ensure you are able to follow your clinical and leadership development priorities identified through your personal development and appraisal plan.

We are achieving excellent results in admission avoidance and reducing the length of hospital stay for the people of Hammersmith and Fulham and are part of a wider three-borough CIS offered in Westminster and the Royal Borough of Kensington & Chelsea. We will equip you for mobile working and actively support flexible working practice.

### Main duties of the job

- The post holder will be an experienced Registered Nurse in Adult Nursing or Paramedic who will work across the Rapid Response Team and Home First Team (H1st).
- The post holder will provide community-based responsive input to patients who are referred with undifferentiated, varied, and complex needs ensuring holistic assessment and community care pathway planning is undertaken to directly prevent unnecessary acute hospital admission.
- The post holder will be a vital clinical resource and a first responder within  2-hour interval, facilitating high-quality evidence-based practice and ensuring patients from a variety of acute/hospital and community settings appropriately access out of hospital community health and social care services.
- The post holder will provide interventions and will make clinical decisions that support patient choice and wishes. They will be expected to work autonomously whilst drawing assistance from other senior colleagues in primary and secondary care.

The post holder is expected to work within the service operation hours Monday to Sunday on an 8 am – 8 pm or 10 am -10 pm shift pattern across the tri-borough sites.

## Person Specification

### Experience

**Essential**

- Experience of teaching and mentoring students.
- Experience of working in a MDT
- Medication management: experience in subcutaneous, IV, IM lines, with administration of medicines in a diverse range of patients.
- Minimum of 3 years’ experience working at Band 5 level
- Experience of managing acute episodes of care, cellulitis, urinary tract infection (UTI), falls, functional, mobility, cognitive impairment.
- Experience of venepuncture.
- Experience of working in an acute setting with high acuity patients i.e. London Ambulance Service, A&E, Intensive Therapy Unit
- Experience in delivering short term episodes of care i.e. from 3 – 7 days
- Experience of prioritising and managing own workload with minimal supervision
- Evidence of own Clinical Supervision/ Reflective Practice
- Experience of safe patient handling

**Desirable**

- Contributing to a quality improvement project.
- Community and rapid response experience
- Experience in advance care planning and End of Life care, identifying and managing clinical situations working within Coordinate My Care requirements
- Experience of leading audit or project.
- Experience of using Patient Record System e.g. Systm1, Rio
- Experience of contributing to audit or research projects

- Knowledge and ability to use NEWS2/SBAR
- Knowledge and ability to identify sensory impairment, the impact it has on activities of daily living and the health and wellbeing of their patients.
- Ability to know when to discharge and who to refer on to.
- Knowledge on how and why to raise safeguarding concerns.
- Knowledge of incident reporting and reviews.

### Qualifications and Training

**Essential**

- Appropriate professional registration e.g. NMC, Adult Nursing/ HCPC Paramedic
- Wound care, catheter management, pressure area care, IV/Subcut administration.

- Preceptorship training. Physical Assessment Skills course I.e. use of stethoscope, percussion, and palpation techniques on examination.
