### Job summary

\<p\>As a Band 6 Physiotherapist, you will join our Early \<span class="single_desc_int_gb_word"\>Supported Discharge (ESD) team based at Gravesham Community Hospital in Gravesend\</span\>. This is a rewarding community role, \<span class="single_desc_int_gb_word"\>supporting patients with neurological conditions, including stroke, to transition safely from hospital to home. You will play a key role in delivering high quality rehabilitation within patients own environments, helping them regain independence and confidence. This is a varied role where you can follow the patient journey, making a real and measurable impact on recovery outcomes\</span\>. You will be part of a friendly and experienced multidisciplinary team, working closely with Occupational Therapists, Speech and Language Therapists, Nurses and \<span class="single_desc_int_gb_word"\>support staff to deliver coordinated, patient centred care\</span\>.\</p\>\<h4\>Package Description\</h4\>\<p\>You will be joining a \<span class="single_desc_int_gb_word"\>supportive and forward thinking Early \<span class="single_desc_int_gb_word"\>Supported\</span\> Discharge team who are passionate about stroke rehabilitation and community based care\</span\>. The team encourages learning, development and \<span class="single_desc_int_gb_word"\>collaboration, providing an excellent environment for Band 6
