Are you an experienced Occupational Therapist looking for a Community role? We have a new and exciting opportunity for a Band 6 Occupational Therapist to join the team based in Thatcham.

You will be an Occupational Therapist with extensive knowledge and experience working with patients with complex physical health needs. You shall take responsibility for the effective running of the Occupational Therapy specialities contribution to the multi-disciplinary team for our inpatient wards at West Berkshire Community Hospital.

### Main duties of the job

Demonstrate a highly skilled knowledge, understanding and ability of how to apply theory to practice, addressing occupational performance and skill deficits, enabling the patient in components of self-care, productivity, and leisure, with a caseload of highly complex and multiple presentations.

Identify appropriate Therapy models, theories and approaches and demonstrate a highly specialised knowledge of evidence-based practice to address the clinical needs for patients with multiple and complex situations.

Undertake the comprehensive assessment of patients with complex physical, mental health and social needs, using investigative and analytical skills to formulate individualised management and treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options to formulate specialised programmes of care.

Work as part of the multidisciplinary team (MDT). This will include case conferences and discharge planning and should ensure that all highly complex patient information is managed professionally with consideration for patient confidentiality and in-line with data protection guidelines.

Resolve and assist team members in resolution of verbal complaints and support the Team Lead, Service Manager and the Therapy Lead in Community Inpatients in the resolution of written complaints.
