Within BSMHFT our community mental health services are well-established and as part of the BSOL community transformation programme, we are looking to increase our Neighbourhood Mental Health Teams with mental health practitioner roles. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Neighbourhood mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan. We are looking for an enthusiastic and dynamic individual who is able to work within a multi- disciplinary team, working alongside GP’s and other primary care professionals who strive to achieve the best outcomes for individuals within the local community. ### Main duties of the job You will be responsible for providing mental health triage, assessment and outcomes-based intervention to meet service user's needs within the Neighbourhood Mental Health Team. You will be responsible for identifying need and best course of advice, support or intervention, promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
