### Job overview

We are seeking a motivated and compassionate Band 6 Discharge Coordinator to join our Acute Services Team, supporting safe, timely and person centred discharges.

You will work collaboratively with multidisciplinary teams, community partners, and external agencies to ensure effective discharge planning from admission through to transition home. The role involves identifying and addressing barriers to discharge, coordinating care plans, and supporting a “Home First” approach while maintaining a strong focus on patient safety and recovery‑focused practice.

This is a key role within the service, offering opportunities to chair meetings, drive service improvement, and contribute to patient flow across all acute wards.

You will also support junior staff and work closely with ward teams, wider SHPU colleagues and contribute to the ongoing development of the Care Transfer Hub closely aligned with our Sheffield City Council partners.

We are looking for a registered mental health professional with excellent clinical, communication, organisational and leadership skills, including a strong understanding of the Mental Health Act and acute care pathways.

### Main duties of the job

The main purpose of the role is to work closely with clinical and operational colleagues within Acute Services and facilitate and ensure comprehensive discharge and management arrangements are in place to enable the clinical team to be able to provide a timely and safe discharge.

The postholder should be a committed, enthusiastic individual who has the skills and drive to lead and work alongside the clinical team, and work with other managers to take the service forward. The postholder will have a relevant qualification and experience.

The postholder will be a member of the leadership team and provide support to the operational delivery of Acute Inpatient Services to deliver agreed operational and strategic objectives using proactive problem solving to resolve issues.

You will work flexibly across our SHPU sites.

## Person Specification

### Experience

**Essential**

- Demonstrate risk assessment and risk management skills with service users and the environment.
- Able to demonstrate an effective contribution to MDT working.
- Demonstrate experience of supervising qualified staff.

**Desirable**

- Demonstrates an ability to work across agencies
- Demonstrate evidence of leadership on a service development issue.
- Demonstrate experience of supervising qualified staff.

### Knowledge and Skills

**Essential**

- Minimum of 12 months experience of working at Band 5 in acute and community mental health services
- Demonstrate sound leadership skills, possessing the ability to direct and motivate staff.
- Demonstrate a thorough working knowledge of purposeful admission (PIPA) and the Acute Care Pathway.

**Desirable**

- Skills in DBT, CBT or PSI
- Demonstrate evidence of good organisational skills.
- Demonstrates good IT skills

### Training and Qualifications

**Essential**

- Professional Qualification and relevant professional registration
- Minimum of 12 months experience of working at Band 5 in acute and community mental health services
- Demonstrate a sound knowledge of mental health legislation and acute care pathways
- Be RESPECT level 3 trained or willing to train

**Desirable**

- Evidence of multi agency working
- Evidence of being part of, or contributing to a service change
- Ability to work in a 24 care setting
