We are a supportive,  dedicated, caring team who strive to ensure we provide excellent care. Working alongside the wider MDT you will be responsible as the Care Coordinator of a caseload.  This will include completing initial assessments and coordination of care needs, planning and implementation of evidence based care to a defined group of older adult  service users and their carers within a defined catchment area/locality, and via a job plan. You will be responsible to provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for the Hub Manager in their absence as appropriate. You will act as a role model and resource for junior/less experienced staff.
