As a Children's Community Nurse (CCN), you will play a crucial role in delivering high- quality nursing care to children in their home environments. Your primary responsibilities will include conducting assessments, developing care plans, implementing nursing interventions, and ensuring proper documentation and follow- up. This is a full-time position, which will also involve participating in a weekend rota and providing support for an end-of-life on-call rota when necessary.

- To maintain a professional attitude and work in accordance with the NMC Code of Conduct at all times acting as a good role model. To be aware of own learning needs and limitations and to maintain professional expertise by embracing the concept of lifelong learning. To keep up to date with mandatory training.
- Maintain accurate, systematic, and timely record keeping including recording of clinical data.
- To work as an autonomous and lone specialist practitioner
- Ensure continuity of care is established and appropriate support available for children, young people and families, with the aim of preventing unnecessary admission or attendance at hospital and facilitating earlier discharge.
- To provide skilled and effective nursing care for children within their own homes.
- To safely and competently carry out a holistic assessment of the well and acutely unwell child/young person within the community setting.
- To confidently formulate and implement plan of care in accordance with nursing assessment undertaken in conjunction with the multidisciplinary team.
- Ensure prescribed medical care is carried out in accordance with medical request including administration of SC / IV / Central line IV treatments, taking of blood samples or specimens as required.
- Provide skilled, evidenced based, sensitive care to children and young people who require end of life care. Providing them with comprehensive care through the 24-hour period.

**Essential**

- Demonstrates motivation and enthusiasm for the clinical specialty.
- Demonstrates a caring and sensitive approach to the needs of patient / parents / guardians / relatives and other staff.
- Car owner – for day-to-day use at work.
- Ability to work flexibly across a 7-day service, including unsocial hours, and 24 hour on-call when required for end of life care.

### Knowledge

**Essential**

- Demonstrate an understanding of normal child development and the sick child.
- Demonstrate the ability to create a trusting relationship with the child.
- Demonstrate an understanding of child safeguarding issues.
- Recognise stress in self and others and demonstrate the ability to take effective action to manage the situation.
- Demonstrates an understanding of clinical risk/governance.
- Able to demonstrate an understanding of professional and personal accountability in line with the NMC code of professional conduct.

**Desirable**

- Knowledge of oncology and chemotherapy

### Experience

**Essential**

- Experience in assessment, planning and evaluation of patient care according to individual needs.
- Experience working with children who have complex health, long term conditions and/or palliative care needs.
- Demonstrate experience of mentoring and understanding of the assessment process for student nurses and any other member of the MDT.
- Recent experience of caring for an acutely unwell child.
- IV competent
- Ability to undertake A,B,C Assessment.
- Demonstrate ability to work with and to use resources effectively as part of the MDT.

- Wound care
- Cannulation & Phlebotomy
