- Undertake holistic client-centred occupational therapy assessments using appropriate Occupational Therapy models of practice and assessment tools working as an autonomous practitioner.

Job Summary:

This may include sensitively conveying distressing news which impacts on their life e.g. Not being able to return to their own home.
- Liaise with members of the multidisciplinary team and others to ensure effective implementation of the client’s ongoing treatment and management.
- To communicate assessment information, treatment plans and clinical decisions to young people, families and professionals.
- Support, advise and provide education and training for clients, formal/informal carers and staff.
- Undertake assessment of the clients home independently or with others to assess and advise about equipment and care needs to facilitate safe discharge and/or effective functioning at home.
- To effectively work alongside the Nursing team, education and multi-disciplinary teams to implement the individual timetables.
- Maintain accurate client documentation working to the agreed format and standards for the rotation area, and to complete discharge information in a timely manner. This will involve use of a patient database in some rotations. Other Duties:
- Maintain continuing professional development (CPD) and keep a personal CPD portfolio.
- Adhere to Trust, service and College of Occupational Therapy professional standards policies and procedures.
- Collect and collate statistical information relating to client activity.
- Undertake other duties appropriate to the grade and in line with the requirements of the post as directed by the Lead Occupational Therapist.
