### Main duties of the job

Are you a passionate Occupational Therapist looking to develop your skills in a supportive and dynamic community setting?

We have an exciting opportunity for a Band 5 Occupational Therapist to join our Intermediate Care Team. This role is ideal for a newly qualified Occupational Therapist or someone looking to build their experience within rehabilitation, reablement and community services.

Working as part of a dedicated multidisciplinary team, you will support people to regain their independence, achieve their goals and remain safely in their own homes following illness, injury or hospital admission.

About the Role

As a Band 5 Occupational Therapist, you will manage your own caseload, carrying out holistic assessments and delivering person-centred interventions that promote recovery, independence and wellbeing.

You will:

- Assess individuals within their own homes and community settings.
- Develop and implement rehabilitation and reablement plans tailored to individual needs.
- Support safe discharge from hospital and help prevent unnecessary hospital admissions.
- Assess functional abilities and support people to achieve meaningful goals.
- Recommend and provide equipment and minor adaptations to maximise independence.
- Work collaboratively with nurses, physiotherapists, support workers, social care teams and other healthcare professionals.
- Support patients, families and carers to develop confidence and self-management skills.
