To carry out comprehensive physical health assessments and interventions for Community Rehabilitation service users who live in the community in Bristol. ### Main duties of the job As a key member of the Bristol Community Rehabilitation Team, you will conduct comprehensive physical health assessments which include various physical health observations, phlebotomy, electrocardiogram tracings and health and wellbeing advice and referrals. You will be a valued part of a multidisciplinary community rehabilitation mental health team. The team works with people with complex mental health problems with a focus on working with people with complex psychosis. The team provides a wide range of rehabilitation interventions including physical health assessment and interventions based on the Lester Tool. The role involves working with people in their own homes and community settings across Bristol and requires independent and lone working. ### Detailed job description and main responsibilities The role will involve conducting physical health and wellbeing assessments and interventions. The efficient and accurate documentation of each assessment and taking any action as appropriate within their scope of practice and within the relevant trust policies and processes for the team. To effectively undertake the role, duties will include: 1.
