To carry out comprehensive physical health assessments and interventions for Community Rehabilitation service users who live in the community in Bristol.

### Main duties of the job

As a key member of the Bristol Community Rehabilitation Team, you will conduct comprehensive physical health assessments which include various physical health observations, phlebotomy, electrocardiogram tracings and health and wellbeing advice and referrals.

You will be a valued part of a multidisciplinary community rehabilitation mental health team. The team works with people with complex mental health problems with a focus on working with people with complex psychosis. The team provides a wide range of rehabilitation interventions including physical health assessment and interventions based on the Lester Tool.

The role involves working with people in their own homes and community settings across Bristol and requires independent and lone working.

The role will involve conducting physical health and wellbeing assessments and interventions. The efficient and accurate documentation of each assessment and taking any action as appropriate within their scope of practice and within the relevant trust policies and processes for the team. To effectively undertake the role, duties will include: 1. Arranging physical health appointments in line with trust policy.

- Developing and reviewing appropriate physical health and wellbeing care plans identifying, organising and signposting to other resources as appropriate.
- Providing defined assessments/ interventions/ activities as part of the physical health appointment within your scope of practice and level or responsibility.
- Completing relevant paper and electronic documentation, reports and letters in line with the associate practitioner role and local processes, in a timely and effective manner.

5.Provide information, advice and signposting to promote good physical health and wellbeing e.g. Smoking cessation, healthy diet, exercise, alcohol consumption, employment support etc.

### Skills

**Essential**

- The ability to form and sustain a working relationship with service users and carers, demonstrating an understanding of when a more assertive approach might be needed.
- Demonstrate well-developed listening skills and effective verbal communication skills, such as being able to engage appropriately with service users, carers, colleagues, statutory and voluntary agencies, both face to face and by telephone.
- Demonstrate well developed written communication skills, with the ability to produce letters, written reports, maintain accurate records and record required statistical information.
- Able to travel freely and flexibly across Bristol for work.
- Able to demonstrate an appreciation of equal opportunities/diversity issues.

### Experience

**Essential**

- Relevant experience of working in any health and social care setting or related voluntary organisation.
- Able to demonstrate a general understanding of mental health issues with a positive, nonjudgmental approach towards people requiring mental health services and an understanding of the principles of recovery.
- Able to articulate a clear knowledge of policy and legislative frameworks within which mental health services are delivered including a person’s rights under the MHA and Mental Capacity Act, relevant to this role.

**Desirable**

- Experience of physical health assessments including taking physical health observations (Blood Pressure, Pulse, BMI, History Taking etc.) and/or blood samples in any related health care organisation.
- Experience of assessing someone’s mental capacity to make an informed decision
- Experience of mental health services, as a user, carer, volunteer or worker.
- Experience of teaching/training individuals or groups

### Qualifications

**Essential**

- GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent

**Desirable**

- Venepuncture/ cannulation trained
- Training in obtaining an ECG reading (ECG placements)
- Physical or Mental Health Qualification/ Training
