You will provide daily operational oversight and coordination of patients progressing through the POA pathway to support continuity of patient flow, elective activity and surgical booking.

Job Purpose The Patient Pathway Coordinator will support the operational management and coordination of patients progressing through the Pre-Operative Assessment (POA) pathway.

You will work daily from the Trust EPR (Cerner) system and support the booking and progression of patients for surgical specialties. This includes ensuring patients are appropriately progressed through the POA pathway, investigations and outcomes are followed up, and patient records are accurately maintained to support timely surgery booking and reduce avoidable delays or cancellations. The role will work closely with POA nursing teams, anaesthetists, surgical specialties, scheduling teams and operational managers to support patient flow and elective activity.

Key Responsibilities Patient Pathway Coordination Monitor and coordinate patient pathways using EPR/Cerner Support booking and progression of surgical patients Identify and escalate delays, missing actions or incomplete pathway steps Monitor patients awaiting: Anaesthetic review Test results GP optimisation Internal investigations MDT outcomes Nursing review Ensure patients are appropriately progressed through POA prior to surgery booking

Booking & Scheduling Support Liaise with surgical specialties and admissions teams regarding patient readiness Support coordination of patients suitable for booking Assist with management of escalation pathways linked to booking pressures

Escalation Management Monitor specialty escalations received by POA Ensure escalations are followed up and progressed appropriately Liaise with nursing teams, anaesthetists, specialties, patient pathway supervisor and service manager regarding outstanding actions Escalate ongoing concerns to senior operational management or clinical staff where required

Communication & Coordination Act as a point of contact between POA, admissions, anaesthetics and surgical specialties Maintain communication regarding outstanding patients and booking readiness Support smooth operational flow across elective pathways

## Person Specification

### Experience / Knowledge

**Essential**

- Knowledge of NHS administrative policies and procedures
- Knowledge of NHS service provision including the pathway of care

**Desirable**

- Knowledge of Trust I.T. systems e.g. EPR, iFit
- Proven experience working in a similar role in a hospital/health care setting.

### Education / Qualification

**Essential**

- Minimum of 5 GCSEs or equivalent experience
