### Job overview

To provide a Social Work Service to a defined patient caseload and their families and carers.

To work in collaboration with the Multidisciplinary team to support the overall care and treatment plan of the individual.

Provide and receive complex, sensitive, or contentious information, where there are significant barriers to acceptance. This could be in a hostile or highly emotive atmosphere dealing with child protection issues, vulnerable adults in a mental health setting.

Use highly developed interpersonal skills to successfully engage with a wide range of patients, families, carers, and staff.

To work with other disciplines and teams within Sheffield Health and Social Care (SHSC) to ensure service users benefit from a multi-disciplinary approach that best matches clinical need.

To work collaboratively with service users, carers, and partner agencies

Maintain accurate and contemporaneous clinical records in line with data quality guidance, legislation, and policy.

Deliver supervision to more junior members of staff.

### Main duties of the job

To plan and arrange subsequent mental health care based on identified needs.

Provide profession-based support and advice across the MDT and within multidisciplinary meetings.

To lead and possess a full understanding of the discharge process in low  secure settings and the knowledge and skills to apply effectively in practice to ensure  a safe and timely discharge takes place.

To ensure that the estimated discharge date is identified at admission, during reviews where possible. Putting in place early interventions to remove barriers to prevent patients discharge process being delayed

To negotiate clinical input from other services within SHSC to ensure the

provision of a clinically appropriate and multi-disciplinary approach

To develop close working links with other mental health teams and agencies

to enable swift and appropriate transfer between services

To share assessments and updates on the person’s health status, including medication information with both hospital and the community based teams

To contribute to the development of time limited individual care plans in

consultation with patients, families/carers, community teams, primary care

and other appropriate service providers

Engage in functional Assessments, Goal setting and group work  as appropriate

To be the main contact person and be confident at building, developing  and fostering  relationships with carers families and friends.
