We are a seven day service consisting of a specialist team of Nurses and Physiotherapists who work primarily to meet the needs of patients with long-term respiratory conditions. The role will include conducting face to face assessment and reviews of both long-term respiratory conditions and acutely unwell people within their own homes, hospital and a clinical environment. We also provide the home oxygen assessment and review service for all adult patient regardless of diagnosis. Post registration experience in a relevant field for a minimum of 3 years is required. ### Main duties of the job You will act as an autonomous practitioner within the specialist team providing both community care and in-reach into the acute trust. This includes a mixture of face to face and telephone assessment of people with Respiratory or long-term conditions. You will be flexible to meet the needs of a reactive service and should be able to prioritise your workload based on patient/service need. You need to be able to perform under pressure within an extremely busy service ### Detailed job description and main responsibilities JOB PURPOSE - To provide the specialist practitioner function in the delivery of specialist care to patients who have been referred to the Integrated Respiratory team. - To provide specialist input into the assessment, monitoring and evaluation of chronic respiratory patients within acute, outpatient and community settings and lead in the provision and running of Oxygen assessment clinics. - To be involved in developing and delivering high quality patient centred care and working with the associated clinical team to ensure this is incorporated within the Trust objectives.
