### Job summary

This role helps people in the community by supporting rehabilitation and daily living, making a real difference to their independence and wellbeing. You'll work as part of a friendly multi-disciplinary team, including occupational therapists and physiotherapists. Your work will help patients recover, stay active and manage long-term conditions. You'll use your skills to deliver care that is safe, person-centred and based on clear plans.

### Main duties of the job

What you'll do:

- Support assessments and deliver agreed care and therapy plans
- Monitor patients' progress and report any changes promptly
- Encourage and motivate people to take part in their care and activities
- Liaise with colleagues, carers and external services to coordinate support
- Maintain accurate records in line with clinical policies

What we're looking for:

- You might have a Level 4 qualification in health and social care or similar experience
- Experience working in a care or rehabilitation setting
- Good communication skills, including with people who have complex needs
- Ability to work independently while knowing when to seek advice
- A flexible, team-focused approach and commitment to high standards of care

## Person Specification

### Knowledge

**Essential**

- Demonstrate awareness of healthy living practices
- Knowledge of the Health and Safety at Work Act
- Knowledge of preparing rehabilitation programmes for improving independence and fitness
- Knowledge of risks associated with performing exercise and when to escalate clinical concerns to the most appropriate clinician.
- Clear understanding of the rehabilitative process and role to play within this.

**Desirable**

- Knowledge of the Mental Capacity Act, Safeguarding Adults and Young People

### Experience

**Essential**

- Experience of working within a clinical care setting
- Experience of engaging people in healthy living practices
- Experience of working with groups of people
- Experience of working in a multi-disciplinary environment

**Desirable**

- Evidence of working within rehabilitation clinical services
- Previous experience working with people with physical needs and adults with complex needs
- Experience of working with people with chronic/progressive disease
- Experience of liaising and working with multidisciplinary teams or care teams

### Qualifications

**Essential**

- Relevant NVQ level 4 or Foundation Degree in Health and Social Care or demonstrable working experience in the field of rehabilitation.
