### Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is inviting Expressions of Interest from doctors who are able to provide trust-wide senior clinical leadership, strategic direction, and transformational oversight for Older Peoples Mental Health pathways and within the healthy ageing and frailty and adult mental health change programmes. This role is integral to shaping future models of care, ensuring patient safety, driving innovation, and developing the conditions for success in terms of culture, workforce and use of resource.

### Main duties of the job

Model effective partnership collaborating with key partners across the dementia and older people's life span.

Working with a clinical network, partners and divisions to drive the development and implementation of new models of care aligned with population health needs and national policy frameworks, taking an integrated life span approach.

Lead, Champion or Co-lead initiatives to eliminate unwarranted variation in clinical standards in OPMH services

Support a positive culture of continuous improvement, innovation, and excellence in care within OPMH and help support the conditions for success.

Champion holistic patient centred approaches; maintaining clinical credibility through working within an older people's pathway.

Work with the OPMH multiprofessional leads, Divisional CDs and their teams across the four geographical divisions and establish professional links with CSS and SAC, in order to contribute to OPMH strategic decision-making and service improvement and promote learning across the geography.

Ensure alignment of cross-divisional OPMH clinical priorities with divisional aims, with a strong focus on patient safety and quality.

Collaborate with AMH colleagues to optimise transitions of care and pursue options for needs led services.

Work collaboratively with the CMO team to deliver the CMO team goals.
