An exciting opportunity has arisen for an Assistant Practitioner with our Early Supported Discharge (ESD) Stroke team. We are looking for an enthusiastic and motivated individual with good communication, organisational, and interpersonal skills, with the ability to work as part of a multidisciplinary team and on your own initiative.

The post holder will need to be able to demonstrate relevant experience of working in the field of rehabilitation within the NHS and/or Social or Community Care settings. Previous experience as a Therapy Assistant is highly desirable for this role.

### Main duties of the job

Support physiotherapists, speech and language and occupational therapists working within the stroke team across Swansea Bay, in the assessment and rehabilitation of patients. The work will be based mainly within the Stroke Early Supported Discharge (ESD) Team and involve community based therapeutic interventions and visits to aid prompt discharge, and assessing patient progress in their home.

### Qualifications

**Essential**

- Recognised qualification in a relevant subject e.g. NVQ3 diploma in rehabilitation support, or relevant experience
