### Job overview

We are looking to recruit two proactive and compassionate Assertive Outreach Support Workers to join our Community Mental Health Hubs (CMHH). One role will be based across Queen’s Park & Paddington hub (Woodfield Road), and one within the South Hub (190 Vauxhall Bridge Road).

You will support individuals with complex mental health needs who are at risk of disengagement, providing practical, person-centred care to promote engagement, independence and recovery.

This is a rewarding opportunity to work as part of a supportive multidisciplinary team, with access to regular supervision, development opportunities, and the chance to contribute to an innovative and evolving service.

### Main duties of the job

This is a varied and rewarding role working directly with individuals with complex mental health needs who may find it difficult to engage with services. Using an assertive outreach approach, you will build therapeutic relationships and work flexibly in the community to support engagement and recovery.

You will provide practical support to help individuals develop daily routines, maintain independence, and engage in meaningful activities such as self-care, social connection, education, or employment. This includes accompanying individuals to appointments, supporting access to services, and helping overcome barriers to engagement.

Working alongside clinicians, you will contribute to care planning, risk management, and delivery of time-limited interventions. You will support early identification of deterioration, contribute to de-escalation, and help reduce the need for hospital admission.

The role involves working with carers and partner agencies, including health, social care, and voluntary services, to promote continuity of care and social inclusion.

We are looking for someone compassionate and resilient, able to engage individuals in a flexible and persistent way, with strong communication skills and a commitment to person-centred, recovery-focused care.
