One role will be based across Queen’s Park & Paddington hub (Woodfield Road), and one within the South Hub (190 Vauxhall Bridge Road). You will support individuals with complex mental health needs who are at risk of disengagement, providing practical, person-centred care to promote engagement, independence and recovery. This includes accompanying individuals to appointments, supporting access to services, and helping overcome barriers to engagement. Working alongside clinicians, you will contribute to care planning, risk management, and delivery of time-limited interventions. You will support early identification of deterioration, contribute to de-escalation, and help reduce the need for hospital admission. The role involves working with carers and partner agencies, including health, social care, and voluntary services, to promote continuity of care and social inclusion.
