The team assess and manage treatment for patients requiring various forms of anticoagulation for a number of clinical conditions. The team have daily clinical contact with patients on anticoagulation therapies and have both face-to-face and telephone consultations with patients about their treatment and blood results to maximise efficacy and reduce the risk of bleeding and thrombosis.
