### Job overview

Alcohol excess plays a significant role in a wide array of health and social care needs. Consequently this impacts and imposes a burden on the whole health economy.

A significant and rising number of patients with alcohol related problems attend our Emergency Departments which may subsequently being admitted. There is a high re-attendance and readmission rate and we have a higher than average alcohol related mortality in Teesside.

Consequently there is a case for improvement in the provision of alcohol related services and better coordination of care together with our community providers.

The North East and North Cumbria ICS support the development of alcohol services and NCIC have made a successful bid for funding to support Alcohol Care Teams. They invite us to join with colleagues within our ICS to pledge support for the development of services to address this pressing need.

As an integral member of the multi-disciplinary team the post-holder will work in partnership demonstrating compassionate caring behaviours to fulfil the key functions of the Alcohol Care Team (ACT).

### Main duties of the job

- The role involves working 12 hour day shifts as part of a dynamic alcohol care team that provides a 7 day services within JCUH.
- Carrying out assessment of patients who maybe drinking alcohol outside of the safe recommended levels, identifying the different levels of alcohol dependant drinkers.
- Providing IBA, PSI and relapse prevention work.
- Referring onto community alcohol support services, also liaising with community alcohol support service to enable a seamless transition from hospital to community in the best interest of the patient.
- Work closely with the ACT specialist nurses.
- Providing patient care from a holistic approach identifying social needs, safeguarding, housing, domestic violence, mental health and liaising with the appropriate services, both in JCUH and externally,  to provide support whilst in hospital which is then carried on into the community.
- Using IT database to accurately record patient records

### Experience

**Essential**

- Experience working with addiction

**Desirable**

- Experience working in mental health

### Knowledge & Skills

**Essential**

- Knowledge and experience in delivering psychological interventions including brief interventions.
- Excellent communicator
- Ability to be flexible and to manage your workload effectively.
- Basic IT skills

**Desirable**

- Knowledge of or experience in coaching and mentoring practices and tools.

### Qualifications & Training

**Essential**

- NVQ in social & Health level 3

**Desirable**

- Psychosocial intervention training
- Relevant CDP/Short courses
