## Job Content

### Job overview

The therapies department at UHNM are looking to support a winter project for 6 months to provide increased therapy cover within the Integrated discharge hub. This is a pilot role that was tested and found to be very successful last winter and is a role that will sit within the integrated discharge hub team (IDH). This opportunity is open to all Occupational therapists and Physiotherapists at band 7 with a keen interest in discharge planning or at band 6 with a focus to develop into a stretch role with a specific interest and focus on discharge. The role will be a great opportunity to work with our MPFT colleagues in IDH and to collaborate over an essential trust project to increase home discharges and promote the therapy role. This role will provide opportunities to experience discharge planning and the role of the social care team as well as exploring the community led services available to UHNM.

The project will be running from October through to March, we would encourage opportunities for job shares and part time working as well as full time applicants.

The project will also involve weekend working and Bank Holidays.
