This is an exciting opportunity to take a senior clinical role across two important mental health services, supporting patients with complex needs while working closely with multidisciplinary teams to deliver safe, effective and person-centred care. ### Main duties of the job - Provide a comprehensive specialist clinical pharmacy and medicines optimisation service to CAMHS and CRHTT patients and their multidisciplinary teams. - Undertake patient-centred clinical medication reviews for people with complex mental health needs, polypharmacy, adherence difficulties and comorbidities. - Provide expert advice on the safe, appropriate and cost-effective use of medicines. - Participate in MDT meetings, inpatient ward rounds, CPA meetings and discharge planning where appropriate, particularly for patients transferring to CRHTT. - Support reviews of pharmacological treatment options and, where appropriate, lead aspects of medication-focused ward rounds or MDT discussions. ### Detailed job description and main responsibilities We are looking for a pharmacist who can demonstrate: - Significant clinical pharmacy experience and a strong commitment to patient-centred care. - Specialist knowledge and experience relevant to mental health pharmacy and medicines optimisation. - The ability to work confidently and collaboratively within multidisciplinary teams. - Excellent clinical reasoning and decision-making skills when managing complex patients. - Strong communication and interpersonal skills, including the ability to work effectively with patients, carers and healthcare professionals. - Experience of supporting and developing colleagues through education, training, supervision or mentorship. ### Applicant requirements You must have appropriate UK professional registration.
