### JOB SUMMARY

The post holder will work as an Advanced Practitioner Level 1 Speech and Language Therapist within the SLT Children’s Community Service.

To be responsible for/contribute to the operational management of the Community Clinic / Clinics.

The post holder will practice autonomously, providing highly specialist clinical expertise in the assessment, diagnosis, management and discharge of children and young people (CYP) with a broad range of speech, language and communication needs, associated with Learning difficulties, Developmental Language Disorder, Significant Speech Sound Disorder and ASD. The post holder will have a clinical caseload.

### KEY DUTIES / RESPONSIBILITIES

To be responsible for comprehensive, specialist, holistic assessment, diagnosis, prioritisation, treatment and discharge of CYP within the client group, in partnership with parents/carers and relevant Health/Educational professionals.

To promote and develop a collaborative approach in the management of these groups of children and young people as the post will have a strong emphasis on inter-agency working, particularly with Education.

Be proactive in clinical decision making underpinned by an advanced level of theoretical and practical knowledge.

To be responsible for the planning and management of the caseload in conjunction with the clinical and service lead and other senior staff as appropriate.

To refer children to Specialist Speech and Language Therapists as appropriate, for further specialist assessment and advice, in liaison with Clinical and Service lead.

To ensure close working with colleagues within Health, Education and Early Years in the planning and implementation of joint programmes.

To provide a point of reference for other professionals as appropriate.

To be proactive in the research and application of evidence-based practice to ensure communication supportive environments, which maximise children and young people’s communication skills and learning opportunities.

Professional, operational and clinical supervision will be provided by the Speech and Language Therapy Clinical and Service Lead for Community Clinics.

The post holder must be prepared to undertake further training as required.

Clinical and Professional Responsibilities

To monitor and evaluate outcomes of clinical practice and co-operate with the Clinical and Service Lead and other senior staff in the development of efficient and effective models of clinical intervention and service delivery.

To develop and extend a level of advanced professional knowledge, clinical practice and expertise relevant to the specified populations.

To work in partnership with other professionals, parent and carers in the delivery of a SLT service which is sensitive to the needs of the specified group of children and young people.

To be responsible for the transfer of children and young people to other Speech and Language Therapy children’s services and vice versa.

To have responsibility for ensuring robust governance within the Speech and Language Therapy clinics.

To support, guide and supervise less experienced staff to develop their competencies.

To plan, design, deliver and evaluate training programmes to parents, carers, teachers, support assistants and other professionals.

To be responsible for the triaging of referrals to the relevant Speech and Language Therapy service as and when required.

To engage in PPI (Personal and Public Involvement) and co-production as a means of developing and delivering services.

Participate in the induction and training for SLTs and SLTAs as required

To provide second opinion to other Speech and Language Therapy staff across South Eastern Trust as appropriate.

To provide advice to Clinical and Service Lead regarding the development of services for the client group.

To work closely alongside and provide specialist Speech and Language Therapy advice and training to other professionals within Education and Health, parents/carers, voluntary agencies and Speech and Language colleagues as required.

To work in collaboration with Education and Health staff in designing and implementing therapy programmes for individual children as required and to provide reports/attend review meetings in schools, as required

To assist the clinical and service lead in the identification of, and take delegated responsibility for, the development and implementation of policies, standard setting, audits and evaluation of service developments within the clinical area.

To provide appropriate advice reports in relation to the Education (NI) Order 1996 and Code of Practice (DENI) within specified time scales.

To adhere to the Royal College of Speech & Language Therapists Professional Standards

To adhere to Heath and Care Professions Council (H.C.P.C.) standards.

To be accountable for one’s own professional actions, recognise professional boundaries and seek advice as appropriate.

To provide clinical placements for all undergraduate students and Speech and Language Therapy Students from Year 1 up to and including Year 3.

To be responsible for continued personal and professional development.

To complete R.C.S.L.T. CPD annually and copy to line Manager.

To attend and participate in Staff Forums, Team Meetings, Project Meetings and Quality Evidence days as appropriate.

To attend and participate in clinical excellence networks (CEN) specific to the specialist clinical areas of work as appropriate.

To liaise with other relevant Health and Social Services staff, Education staff, and non-statutory organisations.

To initiate and participate in case conferences as required.

To participate in regular supervision and annual appraisal with the Clinical and Service Lead.

When required support and facilitate colleagues in research and clinical audit in order to improve effectiveness and quality of patient care.

To provide professional advice to the clinical and service lead on issues relating to clinical and social care governance.

To contribute to the Speech and Language Therapy Department business planning process, as appropriate.

To ensure adherence to the clinical and social care governance guidelines within the Speech and Language Therapy team and inform the clinical and Service Lead of any issues.

To support, guide and delegate duties to less experienced staff or administrative staff as appropriate.
