The post holder will be a key member of the Foot protection team and provide a professionally competent podiatry service to patients in both the community and Acute settings throughout the Locality of the South Eastern Trust, With A requirement to work Trust Wide as need dictates. The Post will have a particular focus on the avoidance of hospital admission and rapid access to the Diabetes hub in the ulster hospital. The post will focus on the management of diabetes and the high risk foot. This role is becoming increasingly specialised with emphasis on wound management and the prevention of serious foot pathologies. The post holder will be expected to use his/her own initiative to develop and interface with the Enhanced Foot Protection team members and the Diabetes Team in UHD. In conjunction with colleagues and podiatry management the post holder will contribute to the development of diabetes services in both Acute and Community clinics.

### KEY DUTIES / RESPONSIBILITIES

Clinical/Professional/Specific Managerial Responsibilities

To work in conjunction with the South Eastern Trust Lead Podiatrist, The Principal Podiatrist for diabetes, The principal Podiatrist for High Risk foot and other professional groups regarding the implementation and on-going development of the Diabetes feet care pathway, evaluating the effectiveness of Podiatry intervention and proposing / implementing standards & policies for practice.

To work collaboratively with the Principal podiatrist for Diabetes to provide expert clinical guidance, support and advice to colleagues and Podiatry management team on all aspects of Podiatry relating the post holders specialist field

To work alongside the EFPT and the Diabetes MDT to prevent hospital and ED admission by providing rapid access to the EFPT as part of an Urgent care centre.

To manage a designated caseload using advanced clinical reasoning to assess how the client’s podiatric needs impact on their ability to function physically in their activities of daily living , physical environments ,make an independent diagnosis and formulate a treatment plan

Integrate treatment programmes into the overall multidisciplinary management of the client where that is applicable

Maintain comprehensive and accurate clinical records in accordance with professional standards and using the appropriate IT systems within the Diabetes service.

Deliver and promote evidence based practice and agreed care pathways

Lead by example in the promotion of high standards of clinical treatment and podiatric care, to ensure a quality service.

Provide guidance and advice to the multidisciplinary team, when appropriate, in the management of complex cases

To work flexibly across various Trust locations to deliver safe and effective care.

To manage difficult clinical situations arising from working with clients with Diabetes related behavioural and podiatric needs.

To allocate clinical work to other Podiatry staff commensurate with grade and monitor the professional standard of treatment provided.

To communicate effectively and collaboratively with service users, relevant MDT’s and other agencies promoting the value of Podiatry for clients

To actively participate and provide appropriate clinical feedback in Podiatry Senior Management Meetings, Podiatry Team meetings, MDT meetings as required, risk management planning, case conferences, serious adverse incident reviews and any other service development initiatives.

To work collaboratively with the principal podiatrist for diabetes in ensuring the successful implementation of the Diabetes Feet care pathway.

Provide Expert clinical advice, support and guidance to the community Foot protection team across trust as required.

Provide cover for the Enhanced Foot Protection Team Podiatrists working in the Acute setting as required.

Continually develop and modernise the Trust Diabetes and High risk foot service by researching, developing and implementing innovative and current methods of working, ensuring continued delivery of evidenced based practice

Generic managerial responsibilities both within an operational and strategic context

Manage client referrals waiting times, implementing appropriate strategies when required

To adhere to the code of ethics and the Society of Chiropodists and Podiatrists Standards, Trust and Professional Policies and Procedures in the discharge of responsibilities.

To maintain stock levels and be responsible for the material stock used within the facility.

Responsible for equipment maintenance, together with monitoring and evaluation of equipment needs / changing patterns

To lead on formation of policies and service developments within agreed professional standards for podiatry within diabetes and High Risk services

To complete statistics and provide timely returns as appropriate.

Evaluate service/ clinical developments; consider options and making recommendations for change / service improvements

Work collaboratively with the principal podiatrists for Diabetes and High Risk foot, and the Podiatry management structure in delivering an effective podiatry service and in the strategic and policy development for the service and podiatry

To participate in multi-professional and uni-professional audit.

To provide specialist expertise on uni-professional audit and research relevant to this specialist area.

To ensure practice is evidence based and remains up to date with current developments by participating in mandatory and professional training.

To identify mandatory and professional training needs of staff and advice the Lead Podiatrist accordingly.

To actively participate in operational and clinical supervision and supervise junior staff and / or students as required.

To participate in recruitment and retention of staff.

To work flexibly, and promote flexible working amongst staff, to meet the needs of the service including evenings and / or weekends if required.
