An opportunity has arisen for a part-time Advanced Clinical Pharmacists to join the Clinical Streaming Hub in Cardigan Integrated Care Centre.

You will be a key member of the Intermediate Care team, working alongside MDT colleagues to support community services. This role offers the chance to apply advanced clinical skills and make a meaningful impact on patient care.

You will deliver high-quality, patient-focused care through consultations, diagnostic assessment (within scope), and comprehensive medication reviews from MDT referrals and home visits. You will also provide expert advice and support complex prescribing decisions.

Applicants must work autonomously, prioritise workload effectively, and commit to ongoing professional development. Strong clinical and communication skills are essential.

We are seeking a forward-thinking, flexible and compassionate pharmacist to help develop this evolving service.

An independent prescribing qualification is essential, with opportunities to develop specialist clinical expertise aligned to service priorities.

### Main duties of the job

As part of a dynamic and forward-thinking team you will:

- Provide expert clinical pharmacy support to the Clinical Streaming Hub

- Provide Comprehensive medication reviews for patients on the caseload (face to face, telephone or in a patient’s home) and prescribe/deprescribe appropriately within scope of practice.

- Work closely with multi-disciplinary teams, including GP practices and community pharmacies to ensure safe and effective medicines use

- Support antimicrobial stewardship and medicines optimisation initiatives

- Provide education and training on medications to other clinical staff across the Clinical Streaming Hub team, as well as providing education and training by supervising and mentoring junior pharmacists, pharmacy technicians, pharmacy students.

- Maintain accurate and thorough records of all patient interactions, using appropriate clinical templates and coding in line with service protocols.

- Leading on clinical audit of medicines prescribed in the service and taking a proactive role in quality improvement initiatives, implementing recommendations where appropriate to enhance patient care.

- Provide patients with clear, independent information regarding their medications and any changes to their prescriptions, referring to other healthcare professionals for additional support when necessary.

## Person Specification

### Experience

**Essential**

- Specialist knowledge of medicines management systems and processes in an integrated NHS
- Specialist knowledge of own field of practice gained through diploma level training and experience
- Relevant clinical experience and knowledge

**Desirable**

- Demonstrate specialist knowledge in more than one field preferably in primary care
- Practice Research Experience
- Training and education of pharmacy staff and other Health Care Professionals
- Experience of managing change
- Experience of service improvement

### Qualifications and Knowledge

**Essential**

- Master’s degree in Pharmacy
- Registered Pharmacist with GPhC.
- Post-graduate diploma/MSc in clinical pharmacy or equivalent qualification or experience.
- Independent Prescriber

**Desirable**

- Designated Prescribing Practitioner (DPP)
- Member of the Royal Pharmaceutical Society (RPS)
- Membership of specialist body e.g. UKCPA
