### Job overview

The post holder will work as part of a multidisciplinary team and will be based in the community.

The team aim to reduce unnecessary hospital admissions and prevent avoidable Emergency Department attendances by providing a holistic multidisciplinary intervention and support to help stabilise patients in their own home or usual residence.

The post holder will work as a Non-Medical Prescriber.

### Main duties of the job

- To complete a full medication review on the patient’s admission to the service, reducing the likelihood and consequences of patient harm from their prescribed medication
- To ensure rationalisation of medication particularly for complex patients with polypharmacy, including rationalising medication timing to aid patient concordance for self-managed medication, and to help reduce the number of care calls required for patients with managed medication
- To complete patient-centred consultations and liaise with a prescriber, or use the skills as a pharmacist prescriber, to change medication regimes
- To educate the patient on their medicines and discuss concordance
- To liaise with the community health care teams about the patient’s needs including ongoing monitoring requirements
- To provide information and refer to other healthcare professionals and services, including community pharmacies and GPs

- Evidence of use of clinical IT systems

### Experience

- Experience in clinical pharmacy services in a UK NHS hospital
- Experience in delivering training and education
- Experience in audit
- Experience of delivering Medicines Management Standards and Policies
