### Job summary

The Vale Medical Practice are looking to welcome a permanent Advanced Nurse Practitioner to our team for 48 sessions per week (flexible and negotiable).

- Optional Extended Access sessions: Monday and Wednesday: 5.00pm to 8.00pm and enhanced hourly rate available

- Paper-light, digital-first practice with streamlined and efficient systems
- Minimal administrative burden with excellent clinical and administrative support
- Streamlined total triage system with pre-consultation work-up and prioritisation, enabling timely access, reduced pressure in clinics, and more meaningful consultations
- Protected time for follow-ups and continuity of care
- Expanding portfolio of onsite outpatient clinics, supporting closer collaboration with secondary care and opportunities in teaching and service design
- Opportunity to undertake Extended Access sessions for additional flexibility and income
- Supportive team culture with regular team-building and social events
- Weekly clinical meetings for shared learning and peer support

## Person Specification

### Experience

**Essential**

- Evidence of ongoing CPD and professional development
- Annual appraisal and revalidation requirements met

**Desirable**

- Experience working as an Advanced Nurse Practitioner in Primary Care
- Ability to independently assess, diagnose, treat and manage patients presenting with acute and chronic conditions
- Experience in managing long-term conditions and preventative healthcare
- Ability to work autonomously whilst recognising limitations and seeking support when appropriate
- Competence in clinical examination, history taking and clinical decision-making
- Understanding of safeguarding procedures and professional responsibilities
- Ability to take responsibility for your own career development, learning and performance and ensure mandatory training remains up to date
- Awareness of and compliance with relevant practice policies and guidelines, including prescribing, confidentiality, data protection and health and safety requirements
- Commitment to lifelong learning, audit and quality improvement to ensure evidence-based practice
- Excellent interpersonal and communication skills with a patient-centred approach

### Qualifications

**Essential**

- Registered Nurse with current NMC registration
- MSc Advanced Clinical Practice (or working towards)
