We are a friendly, supportive practice and this is an exciting opportunity to play an important role in expanding and developing our clinical services whilst improving access to timely patient care. The ANP will work closely with the GPs and nursing team. ### Main duties of the job Key responsibilities: - Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF. - Provide autonomous assessment, diagnosis and treatment for patients presenting with range of acute and chronic medical conditions. - Independently prescribe appropriate medication, evaluate or refer to other specialists if necessary. - Implement and evaluate individual specialised treatment plans for chronic disease patients. - Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health. - Support and participate in shared learning within the organisation. ### About us Stour Surgery is a well-established, friendly GP practice based in Christchurch, Dorset serving over 10,800 patients.
