This position is open to Acute Nurse Practitioners / Paramedic Practitioners, orAdvanced Clinical Practitioners. The non-clinical team at our practice is highly skilled and very efficient, having a fluid working system with our clinicians. We also benefit from having a First Contact Physio, 2 Clinical Pharmacists, 2 Pharmacy Technicians, 3 Social Prescribers, 2 Health and Wellbeing Coaches, a Physician Associate and 2 Mental Health worker on hand to help with the workload. ### Main duties of the job Beyond the expected assessment, treatment and collaboration as required, you will sometimes need to refer patients for investigations and/or the secondary care input, you will ensure the provision of continuity of care including follow-up if required. You will contribute to the writing of Care Plans for those patients at risk of hospital admission where necessary. You will operate within your own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence. ### About us We pride ourselves on providing patient centred care, in a happy working environment, with excellent teamwork throughout the practice.
