### Job summary

We are seeking an experienced, autonomous and patient-focused Advanced Clinical Practitioner (ACP) to join our innovative multidisciplinary team. This is an exciting opportunity to play a key role in delivering same-day care across our Urgent Care Service, Minor Illness Clinics and Clinical Allocation Hub, supporting patients to access the right care, from the right clinician, at the right time.

Working as an autonomous practitioner within your scope of practice, you will provide advanced clinical assessment, diagnosis, treatment and prescribing for patients presenting with urgent and acute healthcare needs. You will undertake face-to-face consultations within our urgent care clinics, deliver telephone and digital consultations, and carry out home visits where clinically appropriate.

The Advanced Clinical Practitioner is a key member of The Middlewood Partnership multidisciplinary team. Working autonomously and within their scope of practice, the post holder will:

- Assess, diagnose, investigate, treat and manage urgent and acute presentations.
- Prescribe appropriately and coordinate referrals and multidisciplinary care as required.
- Undertake clinical triage and assessment of patient enquiries and requests within the Clinical Allocation Hub.
- Support patients to access the most appropriate healthcare professional or service in a timely manner.
- Provide expert clinical advice and assessment via telephone, digital platforms and face-to-face consultations.
- Deliver safe, evidence-based and patient-centred care.
- Contribute to service development, quality improvement and clinical effectiveness initiatives.
- Support organisational objectives through collaborative working across the multidisciplinary team.

- Provide autonomous clinical assessment, diagnosis, treatment and management of patients presenting with acute, urgent and minor illness conditions.
- Deliver safe and effective online, telephone and face-to-face consultations.
- Undertake comprehensive history taking, clinical examination, investigation and treatment planning.
- Prescribe safely and appropriately within scope of practice.
- Provide self-care advice, health promotion and preventative health interventions where appropriate.
- Refer patients to other healthcare professionals and services to ensure timely access to the most appropriate care.
- Support patients in navigating health and care services effectively.
- Maintain accurate, contemporaneous and legally compliant patient records using the clinical system.
- Ensure all assessments and clinical decisions are evidence-based and aligned to current standards and guidelines.
- Undertake home visits for housebound patients and those whose clinical needs cannot be met within a practice setting.
- Provide clinical assessment and management for patients residing in nursing and residential care homes.
- Support delivery of an alternative model to same-day GP home visits across the Partnership.
- Work collaboratively as part of a multidisciplinary team to deliver coordinated and patient-centred care.
- Communicate effectively with colleagues across primary, community, secondary and voluntary care organisations.
- Contribute to the development and implementation of new models of care and service delivery.
- Attend and contribute to multidisciplinary meetings and case discussions.
- Support innovation and service improvement initiatives that enhance patient care and access.
- Participate in clinical audit, service evaluation and quality improvement activities.
- Monitor, analyse and utilise clinical information and reports to improve patient outcomes.
- Recognise, document and report adverse incidents in accordance with organisational policies.
- Contribute to the promotion of patient safety, clinical effectiveness and organisational learning.
- Support compliance with clinical governance frameworks and quality standards.

### Experience

**Essential**

- Evidence of advanced clinical assessment, diagnostic reasoning and decision-making skills.
- Significant post-registration clinical experience within Primary Care, Urgent Care, Out of Hours Services, Community Services, Walk-in Centres, Minor Injury Units or Emergency Care settings.
- Previous experience working within General Practice or a Primary Care Network.
- Experience in managing undifferentiated and acute presentations autonomously.
- Experience of telephone and digital consultations.
- Experience undertaking home visits and care home assessments.

**Desirable**

- Experience working within a Clinical Allocation Hub or similar triage environment.
- Experience leading service improvement, quality improvement or clinical audit projects.
- Experience supporting the education and development of healthcare professionals

### Qualifications

**Essential**

- Registered Nurse, Paramedic, Pharmacist or Allied Health Professional with current registration with the NMC, HCPC or GPhC.
- Master's Degree in Advanced Clinical Practice (or equivalent advanced practice qualification).
- Independent Non-Medical Prescriber Qualification.

**Desirable**

- Additional specialist qualifications relevant to Primary Care or Urgent Care.
- Formal teaching, mentorship or clinical supervision qualification.
- Qualification in Minor Illness and/or Minor Injury Management
