An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our dedicated, dynamic, and friendly team at Marshalls Cross Medical Centre based within St Helens hospital. You may also work within the community including case managing and providing holistic medical care to patients within a busy care home. You will be an integral part of Mersey & West Lancashire’s clinical team supporting our GP surgery and care home residents. You will clinically support patients (both adults and children) with acute and urgent health conditions and with an emphasis on prevention of hospital care and or admission. You will further support our team with professional development, supervision and ensuring the most up to date evidence-based practice is implemented promoting the best patient experience and outcomes. Previous experience within a Primary Care and/or Community setting is essential for this role. ### Main duties of the job The post holder will work autonomously as a senior nurse within the Primary Care/Community environment, reviewing patients with undifferentiated and undiagnosed health problems; triage, taking comprehensive histories, undertaking clinical examinations, diagnosing, planning & implementing car, prescribing medication and making referrals where appropriate.
