We currently have an exciting opportunity for highly motivated and enthusiastic ACP/to work within our Home Visiting service (Team Up) in North Hardwick and Bolsover.

A Band 7 trainee position may be offered to an aspiring Advanced Clinical Practitioner who is committed to undertaking an apprenticeship training programme.

A Band 8a position will be offered to candidates who have already completed an MSc in Advanced Practice (or equivalent) and who can demonstrate contemporary experience working at an advanced level within a relevant clinical environment.

If offered a trainee position you will commit to undertaking an apprenticeship programme comprising a 3-year MSc in Advanced Practice and the development of an ePortfolio to demonstrate your capability across the four pillars.

Trainees will commence on AFC Band 7 payscale, and uplifted to Band 8a on successful completion of the MSc, successful submission of their portfolio to a cross-organisational review panel.

### Main duties of the job

The successful candidate will support patients referred to  the Acute Home Vising service in North Hardwick and Bolsover primarily ensuring patients remain safe in their own home.

As an ACP you will:

- Provide clinical leadership, including direct clinical care, and advice on complex issues
- Establish a cohesive team approach for the ACP workforce development
- Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps
- Assist in feedback for staff appraisals.

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to patients in their own homes.

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of community patients.

You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.

## Person Specification

### Experience

**Essential**

- Experience of leadership role
- Participation in MDT
- Contemporary clinical exp or older people with complex needs
- Involvement in service improvement

**Desirable**

- Practical understanding of primary care/urgent Care and community services

### Qualifications

**Essential**

- Professional registration with NMC or HCPC
- Evidence of post-registration study at undergraduate level (Level 6)
- MSc in Advanced Clinical Practice (or equivalent)

**Desirable**

- Mentor/teaching qualification
- Independent Prescribing or study of pharmacology & therapeutics at post-graduate level (Level 7)

### Skills and attributes

**Essential**

- Mentor or clinical appraiser
- Excellent communication and interpersonal skills
- Ability to drive / access to transport
- Autonomous practitioner
