Are you an experienced Advanced Clinical Practitioner looking for your next challenge in urgent and crisis mental health care?

We are seeking two dynamic, innovative and highly skilled ACPs to join South West London and St George's Mental Health NHS Trust at a pivotal time of transformation. We have opportunities within both our established Richmond Home Treatment Team (HTT) and our brand-new Crisis Access Hub, enabling successful candidates to play a leading role in shaping the future of mental health crisis care across South West London.

As an ACP, you will work autonomously within multidisciplinary teams, applying advanced clinical expertise across assessment, formulation, diagnosis, treatment, prescribing, leadership, education and quality improvement. You will provide expert clinical decision-making for individuals experiencing acute mental health crises, support the development of colleagues and contribute to service improvement and innovation.

### Main duties of the job

Two Distinct Opportunities

Richmond Home Treatment Team

Join an established and highly regarded Home Treatment Team providing intensive community-based support as an alternative to hospital admission. You will undertake advanced mental health assessments, lead treatment planning, provide non-medical prescribing, support safe discharge planning and act as a senior clinical leader within the team.

NEW Crisis Access Hub

This is a rare opportunity to join a flagship service from the beginning. The Crisis Access Hub will provide a single point of access for mental health crisis referrals across Kingston, Merton, Richmond, Sutton and Wandsworth. Working alongside senior clinicians and system partners, you will undertake advanced clinical screening, enhanced triage, risk assessment and pathway decision-making to ensure patients receive timely access to the most appropriate service.

You will help shape a new model of crisis care, working closely with NHS 111, ambulance services, acute hospitals, community mental health teams, Home Treatment Teams, primary care, social care and voluntary sector partners.

### Detailed job description and main responsibilities

Main Duties of the Role

- Deliver expert autonomous assessment, formulation, diagnosis and treatment planning for people experiencing mental health crises.
- Provide advanced clinical leadership within multidisciplinary teams.
- Undertake non-medical prescribing within scope of practice.
- Lead on clinical decision-making for complex, urgent and high-risk presentations.
- Support colleagues through supervision, coaching and clinical leadership.
- Contribute to service development, audit, quality improvement and research initiatives.
- Foster strong partnerships across health, social care and voluntary sector organisations.
- Promote evidence-based practice and innovation across crisis services.
- Support the development of future ACPs and wider workforce education programmes.
- Contribute to improved patient outcomes through safe, effective and person-centred care.

### Experience

**Essential**

- Extensive experience leading to the delivery of care for a diverse population.
- Evidence of mental health comprehensive assessments, diagnosis, formulation and range of treatments.
- Experience working within healthcare services at an advanced level of practice.
- Advanced level understanding of specialist-specific disorders and corresponding evidence based interventions.
- Contemporary knowledge of relevant legislation and healthcare policies.
- Applied understanding of confidentiality and mental capacity.
- Applied knowledge of safeguarding, inclusion, and equality.
- Experience of leading research or quality improvement initiatives and developing and evaluating new ways of working.
- Experience of working age adult community mental health services.

**Desirable**

- Experience writing for publication or presentation to professionals and the public. Research experience.
- Experience in developing a practitioner-led clinic.
- Evidence of autonomous practice.
- Experience in service development.

### Training & Qualifications

**Essential**

- To be a qualified Healthcare Professional.
- To be a qualified, independent prescriber.
- To hold current professional registration with respective professional bodies.
- To hold a PGDip in Advanced Clinical Practice or MSc in Advanced Clinical Practice.
- Portfolio of evidence for advanced clinical practice consolidation.
- Evidence of continuing professional development.

- Evidence of continuing professional development.
- Postgraduate certificate in education.
