Looking for Advanced Care Practitioners who have a background in unscheduled Emergency/Urgent Care

We are building our team of both ECPs & ACPs creating a dynamic workforce. This is the perfect opportunity for a qualified ACP to join our team. You may be tired of the intensity of ED and enjoy the UCC environment as an alternative.

### Main duties of the job

The Advanced Clinical Practitioner (ACP) will use a high degree of autonomy and complex decision making alongside demonstrating the four pillars of practice, Clinical Practice, Leadership and Management, Education and Research in providing healthcare that enables innovative solutions to enhance experience and improve outcomes within their speciality area.

The ACP is accountable for independent clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice. In addition to this the ACP is also responsible for appropriately referring patients to relevant specialities for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge.

This role covers a rotational shift pattern to include Days, Nights, weekends an bank holidays.

The ACP is accountable for independent clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice.

In addition to this the ACP is also responsible for appropriately referring patients to relevant specialities for any necessary investigations as well as facilitating safe patient discharge.

### Detailed job description and main responsibilities

Demonstrate and promote the Trust’s values and behaviours to the highest standard.

- Be a registered healthcare professional and hold a MSc in Advanced Practice or recognised equivalent by NHS England Centre for Advancing Practice.
- Be an independent Non-Medical Prescriber registered with the NMC, HCPC, GPhC or other relevant professional regulator.
- Practice at an advanced level that is underpinned and demonstrates the four pillars, Clinical Practice, Leadership and Management, Education and Research.
- Demonstrate in depth knowledge and competence in all aspects of Urgent and unscheduled Care encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.
- Provide expert professional and independent clinical care which enables the coordination of a multi professional seamless service for patients.
- Draw on a diverse range of knowledge in their decision-making to determine evidence based therapeutic interventions, which will include prescribing medication as per the Royal Pharmaceutical Competency Framework, trust non-medical prescribing policy, national and local formularies. Alongside providing mechanisms to actively monitor the effectiveness of therapeutic interventions.
- Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, and provide direct support to patients and clients.
- Communicate effectively with patients, carers, colleagues, and others using appropriate communication styles. Ensure timely, accurate and complete documentation reflective of Advanced Practice, consistent with legislation and adhering to local and national guidelines.
- Anticipate barriers to communication and ensures patients and significant others are kept fully informed and consent to treatment.
- Act as a resource for staff, advising on local, national and Trust policy, procedures and in line with professional regulatory bodies and guidelines ensuring patient safety and clinical governance.
- Provide expert clinical leadership and promote excellence in patient care through the provision of specialist advice, support and education to multidisciplinary colleagues across the Trust.
- Work towards health promotion and prevention and comprehensively assess patients for risk factors and early signs of illness.
- Work in partnership with patients, families and carers and support the individual to proactively manage their wellbeing and life.
- The role will include developing audit and research projects and participating in up-dating and implementing departmental policies, protocols, and guidelines, in line with national guidance and contemporary evidence.
- Undertake the role in accordance with Trust and Departmental guidelines but have freedom to act within broad policies and protocols, in accordance with professional responsibilities and boundaries.

Can work autonomously and safely in managing risk, uncertainty and medicd complexity Excellent knowledge base in the assessment, diagnosis, management and treatment of patients who present with undefined injury and/or illness Management experience Recent/ up-to-date kmwEge and continuing evidence of developing practice. In depth understanding of local and national issues eating to emergency or acute care Good decision making, communication and organisationd skills
