### Knowledge

**Essential**

- High level of clinical knowledge including current evidence-based practice
- Highly competent to assess patient’s requirements to develop case management plans.
- Ability to interpret data from diagnostic tests to adjust interventions appropriately
- Knowledge of chronic disease management
- Sound understanding of the expectations of an advanced practitioner, particularly in terms of service outcomes and their relevant context.
- Good understanding of systems in health and social care that are wider than immediate role.
- Demonstrate an awareness/understanding of the factors that contribute to good health and the public health priorities of East Lancashire Hospitals Trust
- Able to plan proactively, manage and evaluate own learning, to negotiate learning opportunities and establish learning contracts
- Demonstrates excellent written and verbal communication skills
- Ability to communicate complex information where agreement and co-operation is required with clients, carers, staff, colleagues and other professionals.

- Evidence of experience in mentoring/teaching role.
- Evidence of experience working in partnership and collaboration

### Qualifications

**Essential**

- Post Graduate Qualification in specific speciality or equivalent experience
- Successful application to MSc in Advanced Clinical Practice.
- Recognised teaching or mentorship qualification at degree level or equivalent
- Leadership module or equivalent Leadership study days
