This is an exciting opportunity to work across inpatient, outpatient, community and virtual ward settings, providing expert assessment, symptom management and care for patients with complex palliative and end-of-life care needs. The role includes supporting home visits as part of our innovative Palliative Care Virtual Ward, helping patients receive high-quality care in their preferred place of care. As autonomous practitioners, ACPs work collaboratively with consultants and the wider multidisciplinary team, acting as clinical leaders and expert resources. We are looking for clinicians with strong advanced assessment and decision-making skills, excellent communication abilities, and a passion for delivering person-centred care. The role also offers opportunities to contribute to service development, quality improvement, audit, research, education and training. Informal visits and discussions are warmly welcomed and encouraged. ### Main duties of the job - Assess patients and plan, implement and evaluate care using specialist clinical knowledge and skills - Review patients and provide care in inpatient, outpatient, Virtual Ward setting and home visits - Contribute to multidisciplinary approach to patient management - Provide education and support to patients, carers and colleagues - Participate in audit, research and service development within the Specialist Palliative Care service. ### Detailed job description and main responsibilities - Perform independent specialist assessment of patients to formulate a diagnosis, plan and implement care and arrange ongoing follow up as an advanced practitioner using highly developed specialist clinical knowledge advanced clinical skills. - Independently manage a patient caseload providing expert specialist assessment, planning and evaluation. - To lead the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of care - Act as a role model and expert clinician providing in depth specialist clinical knowledge to colleagues through MDT meetings, for patients and carers / relatives. - Act as an innovative, enthusiastic role model providing leadership, guidance and advice to staff on operational and professional issues promoting an open and honest and transparent culture. - To improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed. - To provide a consistent and personalised service by ensuring that the patients receive the best care, in the best place from the best person or team. - To provide social, emotional and psychological support for patients from referral and throughout their treatment. - To deliver expert specialist evidence based care including: development, assessment, planning, implementing, evaluating programmes of evidence based nursing care for a defined caseload. - To make appropriate referral to consultants/other specialist/members of the multidisciplinary team in primary, secondary or tertiary care settings. - To be aware of risk management issues both locally and nationally and develop strategies to address identified issues. ## Person Specification ### Other **Essential** - Supportive of colleagues - Enthusiastic and motivated with the ability to motivate the team - High level of personal and professional credibility - Act as role model for nursing staff ### Skills **Essential** - Excellent communication skills - Committed to delivering high standards of patient care - Basic IT skills, including e-mail and word processing ### Knowledge **Essential** - Understanding and application of healthcare governance - Has an up-to-date Advanced Clinical Practice portfolio ### Experience **Essential** - Demonstrable relevant experience within the speciality - Evidence of effective management - Evidence of effective leadership skills - Evidence of staff development - Evidence of change management - Evidence of application of current clinical service issues and developments ### Qualifications **Essential** - Clinical Professional Qualification.
