Are you an ACP,  who wants to work within a community based MDT, having a passion for supporting patient care and keen to make a difference?   Do you want to support the development of the new Community Urgent Care Division?  We are looking for an enthusiastic  Advanced Clinical Practitioner who wants to join our Care Home Support Team and support patients to stay at home, reducing the need for hospital admission.

The ACP will  primarily work within the Care Home Support Team but may be asked to rotate between our Hospital at Home  service,  IV Team as well as the Discharge to Assess team.  The Advanced Practitioner role within the Care Home Support Team will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe and evaluate.

Working as part of a multidisciplinary team and across the primary, secondary and social care interfaces in Manchester, the post holder will be responsible for co-ordinating the delivery of urgent health care needs of people at risk of hospitalisation or requiring a facilitated timely discharge and supporting a more intensive level of care in the intermediate care environment.

This includes direct referrals from NWAS (ambulance service) acute and primary care as well as providing community support.

### Main duties of the job

The post holder will provide specialised triage, assessment and input as an independent practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in a variety of health care settings including Manchester Integrated Neighbourhood teams, Intermediate Care Services, Out of Hours Service, community beds and primary care.

This post holder will also function as the Team Lead, and will have protected time to undertake any necessary management tasks.

The focus will be to prevent unnecessary acute hospital admissions and  enable patients to remain safely and supported at home and prevent premature admissions into long term care facilities.

To assist patients to proactively manage and anticipate their future health needs and develop appropriate plans, in conjunction with their GP and other medical staff as well as Allied Health Professionals, Nursing and Social Care staff to maximise their independence and develop health  plans to manage future health problems and support self-care.

The post holder will lead in developing, delivering and evaluating high quality patient centred interventions in conjunction with the associated clinical teams to ensure this is incorporated within both the overarching Community Urgent Care Division and the Manchester Local Care Organisation’s objectives.

### Experience

**Essential**

- Substantial post registration experience.
- Leadership experience in a a healthcare setting
- Proven experience of working as an independent practitioner
- In-depth knowledge of recent NHS legislation, relevant to community
- Experience of leading, planning and completing audit/research
- Experience of teaching and mentoring of staff across the multidisciplinary team
