This role combines advanced clinical practice with clinical leadership; you will provide line management and professional supervision for the ACPs working within the service together with leading and coordinating the educational supervision of trainee ACPs. You will provide advanced clinical care to patients with frailty, applying expert clinical reasoning and collaborative decision-making. You will contribute to the Trust-wide ACP strategy and frailty pathway development alongside supporting business planning, service redesign and efficiency initiatives. The post holder will provide leadership and oversight to ensure Advanced Clinical Practitioners maintain professional standards and appropriate evidence for revalidation in line with national frameworks. The post holder will lead and coordinate the educational supervision of trainee Advance Clinical Practitioners, supporting staff with educational programmes.
