### Job overview

An exciting opportunity has arisen for an enthusiastic, motivated and committed Advanced Clinical Practitioner with strong clinical leadership and facilitation skills has arisen to join the Intensive Home Support Service (IHSS).

IHSS is an award-winning multidisciplinary team comprising ACP's, Nurses, Physiotherapists, Occupational Therapists, Nurse Associates/Assistant Practitioners and Medics. - Provide clinical oversight and input to patients on the IHSS caseload as well as participating in a number of collaborative projects with Royal Blackburn Teaching Hospital Emergency Department and also NWAS to maximise the 2-hour UCR capacity across Pennine Lancashire.
