### Job overview

An exciting opportunity has arisen for an advanced clinical practitioner to join the evolving Acute Frailty Team at Imperial College Healthcare NHS Trust. The Acute Frailty Team currently provide an in-reach service into the Emergency Departments (ED) at both our St Mary’s and Charing Cross hospitals, assessing patients using a comprehensive geriatric assessment (CGA) framework, with the aim of admission avoidance.

The successful applicant will have experience working with frail patients, ideally within specific frailty services. The role will involve specialist assessments to expertly meet the needs of this complex cohort of patients. Most importantly, the successful applicant will have a passion for older people’s care.

Please note that this position requires the successful applicant to be able to cover both the St Mary’s and Charing Cross sites. Currently the service does not operate at weekends and bank holidays. This is subject to change as the service expands.

### Main duties of the job

- Working as an integral member of the multidisciplinary team (MDT), the post holder will offer expert assessment, treatment and management particularly in relation to frailty syndromes, ensuring high-quality and individualised care for patients, their families, and carers.
- They will lead on comprehensive geriatric assessments (CGA) for appropriate patients, obtaining relevant social and medical histories from patients, families, care providers and 24‑hour care facilities.
- They will exercise professional accountability in all aspects of frailty care delivery, working independently within scope of practice and collaborating effectively with the wider MDT.
- They will support patient flow through early intervention and identifying patients who are suitable for admissions avoidance, as well as those who would benefit from an admission with early identification of their needs.They will also be actively involved in staff education and development, including utilising appropriate learning opportunities and to act as a mentor and resource for student nurses, adaptation and unqualified members of the nursing team.

### Applicant requirements

### Skills

**Essential**

- Advanced theoretical and practical knowledge of elderly medicine, with a wide range of approaches to the management of complex frail patients
- Ability to lead a team of healthcare professionals.
- Experience of running audits and implementing innovative practices

**Desirable**

- Clinical leadership at a senior level in an acute setting including people and change management

### Experience

**Essential**

- Able to work as an autonomous practitioner
- Experience assessing patients using a CGA framework
- Broad clinical experience relevant to the post
- Experience treating older people in settings such as ED/SDEC/acute medicine and urgent community response
- Experience of managing investigations and resolving complaints

**Desirable**

- Experience of managing and running clinics

### Education/ qualifications

**Essential**

- BSc in Nursing or other relevant profession
- Registration with the appropriate healthcare governing body
- MSc in Advanced Clinical Practice

**Desirable**

- Certificated speciality courses
