We are seeking an exceptional Advanced Clinical Practitioner to join our specialised team, focusing on assessment, management and care of patients experiencing dementia and delirium. This is a fantastic opportunity to lead and shape the future of dementia and delirium, whilst working with a dynamic and committed team of clinicians, AHP’s and operational teams across the Department of Medicine for Older people. As an Advanced Clinical Practitioner, you will operate as a clinical expert, providing high – quality person centred care for patients with complex cognitive needs. You will work autonomously and collaboratively across multi-disciplinary teams to improve patient outcomes, reduce lengths of stay and prevent avoidable admissions. This post may be offered under Annex 21 AfC Terms & Conditions, subject to experience, competency and suitability. ### Main duties of the job The post holder will be an integral part of the clinical management team, demonstrating advanced autonomous clinical practice skills across MWL, the post holder will act as the lead for the Dementia and Delirium team across MWL Acting within their professional boundaries, they will demonstrate safe, clinical decision-making skills and will manage their own caseload including the treatment and care of patients from initial history taking, assessment, diagnosis, treatment, and evaluation of care for people presenting with Dementia and / or Delirium. key responsibilities: - Expert Clinical Practice – Conduct comprehensive advanced assessments for older patients presenting with cognitive impairment, formulate evidence-based management plans for complex dementia patients. - Leadership and Innovation - Act as a clinical role model, driving quality improvement initiatives and implementing best practice guidelines across the service & provide line management for the D&D team across MWL. - Education & Mentorship – Provide Specialist training, education and mentorship to nursing staff, allied health professionals and medical teams to enhance organisation wide understanding of cognitive impairment. - Collaborative Care – Partner closely with families, carers psychiatric liaison teams and community services to ensure seamless, holistic patient care. ### Detailed job description and main responsibilities Please find attached Job Description and Person Specification for main responsibilities and requirements. ### Applicant requirements You must have appropriate UK professional registration.
