The role involves working within a community nursing team, supporting the team to offer a high level of care to patients in the place they call home. The post holder will be part of the Urgent Community Response service, visiting patients who are acutely unwell and assessing their clinical and functional needs to try to reduce unnecessary hospital admissions. They will also be involved in the development of the virtual wards, working closely with hospital colleagues. As this is a qualified ACP position we are unable to accept applications for those seeking the Trainee ACP course on this occasion. Hours per week: 22.5 We welcome applications from Graduate visa holders; it is important to note that you must have a minimum of 6 months before this expires at the start of your employment for training and operational reasons.
