### Job summary

Are you an experienced clinical pharmacist looking for an opportunity to develop advanced specialist practice across a diverse and interesting clinical portfolio? We are looking for an enthusiastic and motivated Advanced Clinical Pharmacist (ACP) to join the pharmacy team at Chapel Allerton Hospital, Leeds Teaching Hospitals NHS Trust. Chapel Allerton provides a unique mix of specialist services, and this post offers the opportunity to work across adult rheumatology, dermatology and inpatient clinical pharmacy, alongside a significant role in the management and validation of specialist and high-cost medicines supplied through homecare services. The post holder will develop expertise in the pharmaceutical management of patients receiving complex therapies, including DMARDs, biologic medicines and other advanced therapies used within rheumatology and dermatology. You will work closely with consultants, specialist nurses, pharmacy colleagues and other members of the multidisciplinary team to ensure medicines are used safely, effectively and in accordance with local and national guidance. Alongside specialist rheumatology and dermatology practice, you will provide advanced clinical pharmacy support to the inpatient wards at Chapel Allerton Hospital. These include services caring for patients undergoing complex and neurological rehabilitation, rheumatology and dermatology treatment, elective orthopaedic surgery and stroke rehabilitation.

### Main duties of the job

You will provide an advanced clinical pharmacy service across Chapel Allerton Hospital, with particular responsibility for supporting adult rheumatology, dermatology, homecare medicines and inpatient clinical services. You will work with a high degree of professional autonomy and use advanced clinical knowledge to assess patients, optimise treatment and provide specialist pharmaceutical advice to the wider multidisciplinary team. The role will include: Providing advanced clinical pharmacy input to patients within rheumatology and dermatology, including patients receiving conventional and specialist systemic treatments. Developing specialist expertise in medicines used to manage inflammatory and autoimmune disease, including DMARDs, biologic medicines, targeted therapies and other high-cost treatments. Identifying and resolving complex issues relating to homecare prescriptions, including dose changes, treatment interruptions, monitoring requirements, adverse effects, interactions and continuity of supply. Working closely with the rheumatology and dermatology multidisciplinary teams, specialist nurses, prescribers, pharmacy homecare teams and homecare providers to support the safe and timely supply of specialist medicines.
