### Job overview

The Interim Care and Support Team (ICAST) is a new team being developed to help discharge patients home from hospital sooner.

Your role will be to support the development of the team whilst ensuring  it operates smoothly, efficiently and effectively.

Responsible for supporting the day to day administration and co-ordination of the team and their patients, you will be supervised by the Team Lead and receive regular 1:1 supervisions, annual appraisal and ongoing professional development relevant to your role.

If you have a strong background in team administration and a passion to develop services, build relationships and support others then this is the role for you.

You will be involved in co-ordinating patient discharges and ensuring the operations of a large team are managed efficiently.

Visa Sponsorship: This position is not eligible for Skilled Worker visa sponsorship. Applicants must already hold the right to work in the UK for the full duration of the contract.

### Main duties of the job

- To support the Team Lead with the effective running of the team.
- To maximise the effectiveness of the service.
- Day-to-day administrative management and co-ordination of the service supporting the clinical team and being the first line of contact for patients, relatives, carers and other key stakeholders.
- To co-ordinate staff resources, and admissions and discharges to/from the caseload.
- To work across relevant Trust sites to support the delivery of the clinical service.

### Detailed job description and main responsibilities

- To support the team in the day to day operational running of the service.
- To support and co-ordinate the lone working processes.
- To be a central point of call for staff.
- To liaise with key partners making onward referrals, liaising with key stakeholders, patients, relatives and carers.
- To source additional patient information when required both verbally and electronically.
- To maintain contemporaneous patient records according to team systems; including entering patient information, referrals, documenting contacts and scanning/uploading paper records to the electronic system.
- To maintain contact with patients on behalf of clinicians as requested.
- To be an active part of handover and MDT and support the administration of these.
- To prepare patient information, folders and paperwork needed by clinical staff.
- Undertake stock take and manage stock control.
- Undertake regular audits

### Skills

**Essential**

- Able to make decisions and solve problems quickly
- Uses own initiative and take responsibility to resolve issues
- Able to communicate effectively both verbally and through documentation
- Computer literate

### Experience

**Essential**

- Experience as an administrator in a similar role
- Experience working in complex situations

**Desirable**

- Administrative experience in a healthcare setting

### Qualifications

**Essential**

- GCSE or equivalent in Maths and English (grade A-C)
- NVQ level 3 in administration or equivalent, relevant to the role

**Desirable**

- NVQ level 3 in health and social care or equivalent.
