The role is diverse, including taking referrals, being part of SDUC/Outreach discussion, liaising and arranging services.  You will be working with a number of different professionals in a busy, supportive environment.  Some knowledge of health and social care services would be beneficial as you will be dealing with diverse referrals and pathways, gathering information and coordinating responses.
