Are you an experienced clinical pharmacist looking to expand your skills and clinical expertise by providing an enhanced pharmacy service to an Acute Medical ward within the Acute Medicine and Rehabilitation directorate? If so, we have an opportunity for you in the Oxford University Hospitals NHS Foundation Trust. In your application, you should state the date you will received final ratified university certificate and diploma results if you have not completed.

We are looking for a intuitive, enthusiastic and versatile pharmacist to join our diverse and inclusive team to help us deliver excellent compassionate care to our patients via a patient focused clinical pharmacy service.

The pharmacy department at OUH comprises of over 100 staff in the clinical pharmacy team, and we work across all 4 hospital sites.

This role is based at the John Radcliffe hospital within the Acute medicine pharmacy team. We are continually trying to improve the service we provide, learning from each other to optimise what we do and adapting to changing service needs.

### Main duties of the job

- To provide a ward-based clinical pharmacy service according to Trust agreed standards.
- To maintain a clinical expertise and develop particular areas of interest as appropriate and in accordance with the Trust and Pharmacy strategy.
- To provide clinical pharmacy cover to other ward areas within the team on a reciprocal basis.
- To ensure medicines are used appropriately, safely and cost-effectively in accordance with Trust policy, standard operating procedures and medicines legislation.
- To monitor medicines use within the ward area and contribute to clinical governance plans.
- To work collaboratively with the multidisciplinary team in developing the pharmacy service.

### Experience

**Essential**

- Direct patient care in more than one one setting or clinical area.
- Working with other health care professionals
- Communicating complex information via multiple methods to a variety of recipients.
- Quality improvement, service evaluation or audit
- Mentoring and training others in a healthcare setting
- Taking the lead in a team or project

**Desirable**

- Hospital experience in more than one setting or clinical area
- Experience in Pharmaceutical care of patients in critical care or enhanced care setting
- Appreciation of funding streams for medicines budgets.
- Demonstrating measurable outcomes from quality or service improvement
- Preparation of protocols or guidelines

### Qualifications

**Essential**

- Masters Degre (4 Year MPharm or equivalent) in Pharmacy
- Registered as pharmacist with the GPhC
- Post Graduate Certificate in Pharmacy Practice and due to complete the diploma, or equivalent external credentialling via CPPE/ RPS

**Desirable**

- Post Graduate Diploma/MSc in Pharmacy Practice or experience/ qualification/ credentialling deemed equivalent.
- Independent Prescriber
- Completion of formal Leadership program
