The opportunity has arisen to lead the community based Acute care at home team for the North and East locality of Cornwall. This role offers an exciting opportunity to lead a team of senior nurses providing support and treatment to people in their own homes.  The Acute care at home team deliver treatments which include the administration of IV medications and the monitoring of people during an acute episode of illness to enable patients to recover at home safely. The team will support early discharge from the acute settings, whilst also providing care to prevent a hospital admission where possible. The team work closely with the Virtual wards and Hospital at Home teams to identify people requiring this care. The team leader will be responsible for the professional and operational leadership, for quality and governance within the team ensuring the service is cost effective and efficient whilst also seeking opportunities for team and service development.

To be an autonomous practitioner responsible for the assessment and provision of treatments for patients on the caseload. To manage the performance, and clinical effectiveness of the team ensuring the delivery of evidence based care. To be able to provide assurance that performance standards are being met whilst meeting service targets. To identify new opportunities for service development.

### Skills and Aptitude

**Essential**

- Liaison and negotiation skills, as well as highly developed communication skills
- Highly developed organisational skills

**Desirable**

- Budget Management skills

### Knowledge and ablilities

**Essential**

- Ability to lead, manage and co-ordinate teams in an autonomous manner
- Highly developed specialist assessment and evaluation skills
- Awareness of national and local political health development and influence

### Education / Qualifications

**Essential**

- First level registration adult nursing and registered with NMC
- Experience of implementing change
- Extensive experience of practicing within the community, as an Acute care at Home Nurse

**Desirable**

- Post registered qualification in Nursing the Acutely Ill patient
- A teaching qualification eg City & gilds, HEA Mentorship Module 369
- Experience leading an Acute Care at Home Team
- Specialist or Leadership qualification within a community team
