12 month maternity leave cover beginning August 2026

An exciting opportunity to join the Discharge to Assess (D2A) team at Barnsley NHS Foundation Trust for a 12 month secondment.

The established service provides a clear pathway from the acute hospital by assessing the patients therapy and care needs in their own home. This means that the patients no longer need to wait in hospital for these specific assessments which will reduce delays in discharges, improving patient flow and allow patients to go home earlier.

The team is based at Kendray Hospital therapy hub, involves interdisciplinary working alongside health/social community care and therapy support services. The post holder would be expected to work within 08:00 18:00 service and contribute to a 7 day service. They should have leadership and team working skills, and possess excellent interpersonal and communication skills to work collaboratively with colleagues and patients.

The team fosters a supportive environment that seeks to develop therapists and provide them with the skills to work within the D2A model in the home environment.

We offer clinical supervision, staff support, professional development opportunities and a supportive and friendly working environment.

### Main duties of the job

You will be responsible for undertaking needs-based assessments in the community to identify equipment and care needs at home and amend as necessary. This would include initiating early goals for patients under the D2A pathway, including activities of daily living tasks, falls prevention and progression in mobility.

You will be responsible for the supervision of band 5 rotational staff, band 3 OT assistants and students. Additionally, you would be responsible to co-ordinate the team ensuring patients receive a assessment on the same day as leaving the hospital.

You will work collaboratively with community services including health, social and voluntary sectors to set up packages of care. The postholder will work autonomously within the professional guidelines and the overall framework of the services policies and procedures.

****Due to the nature of the role the successful candidate would be required to be a car driver with access to a car for work purposes****

### Experience

**Essential**

- Post qualification experience (i.e. Band 5 junior grade rotation/static band 5 post), in a relevant field
- Broad range of clinical experience in a variety of OT settings, e.g. must include NHS acute hospital setting
